Eligibility and Enrollment

Becoming a member is your choice.
It’s FREE and your insurance providers or social service benefits will not change.

Here is how to qualify:

• You must be a resident of the Bronx and/or receive provider services in the Bronx
• You must have active Medicaid
• You must have at least one risk factor AND either:
  • Two or more chronic conditions
    OR
  • One single qualifying condition such as:
    • HIV or AIDS or
    • Serious Mental Illness (SMI) (Adults) or
    • Serious Emotional Disturbance (SED) (Children) or
    • Complex Trauma (Children) or
    • Sickle Cell Disease
