Applicants for a Fellowship in Geriatric Medicine are encouraged to submit a detailed current CV to T.S. Dharmarajan, MD, at the above address, along with copies of USMLE score reports, medical school diploma and transcript, residency training, valid ECFMG certificate, proof of visa/citizenship, two passport-size photographs and three letters of recommendation (one must be from the program director).
