In accordance with New York state law, there is a per page fee ($0.75 per page and $6.50 for medical records on CD) if the medical record is being released to the patient or patient representative. If copies are going directly to a physician or hospital, there is no charge.
Patients and authorized patient representatives should allow 7 to 10 business days to process the request.

If Radiology images are needed please contact the Radiology Department at 718 920 6697 and they will assist you on how to obtain the images requested.
If Radiology reports are needed it can be requested by mail or in person by visiting a Montefiore Hospital site Health Information Management Department.
A request to amend records can be done in person by visiting a Montefiore Health Information Management (HIM) Department.
