Montefiore,s innovative Lung Cancer Screening Program has resulted in more than 2,200 patient screenings, with 55 cancers detected over the past seven years, of which about half were more treatable Stage 1 and 2 cancers. When lung cancer is detected and treated early, the five-year survival rate can be as high as 90 percent, but three-quarters of patients are not diagnosed until the disease has spread, reducing their chance of survival to only about 5 percent.

The Lung Cancer Screening Program focuses on smokers and former smokers, age 55 to 74, with a history of at least 30 pack-years of smoking. Created by Montefiore physicians working across multiple disciplines, the program works with patients to identify those at high-risk and provides automated enrollment forms and follow-up reminders to be screened for lung cancer, as well as streamlining the process for referring physicians.

The Montefiore Einstein Center for Heart and Vascular Care is participating in a national study of AccuCinch, a new device to treat an enlarged left ventricle, a condition that affects about half of all heart failure patients. Researchers implant the circular AccuCinch device inside the ventricle using a catheter, then slowly cinch it to reduce the enlargement of the ventricle. AccuCinch supports and strengthens the heart wall, thereby improving heart size and function, and has been shown to slow or halt the progression of heart failure. Ulrich P. Jorde, MD, Section Head, Heart Failure, Cardiac Transplantation and Mechanical Circulatory Support, Montefiore, and Azeem Latib, MD, Medical Director, Structural Heart Interventions, Montefiore Einstein Center for Heart and Vascular Care, are leading the investigations and have successfully completed cases for two different indications.

Montefiore was recognized as Employer of the Year by ARC of New York and its local chapter AHRC, an organization that advocates for and provides services to people with intellectual, developmental and other disabilities. Five years ago, Montefiore became the first New York City hospital to host Project SEARCH, a work transition program that includes on-site classroom instruction, career exploration and job skills training through internships. This year, 12 students are enrolled in Project SEARCH on the Moses, Wakefield and Einstein campuses as part of Montefiore,s Community Workforce Programs. Students can be found sterilizing scopes, making beds, transporting patients, working in environmental services and stocking supplies. Since Project SEARCH began, Montefiore has hired eight graduates. "We are very proud of the accomplishments of the young people in our program, especially achieving competitive employment with family-sustaining wages," says Dana Politis, Director, Community Workforce Programs, Montefiore.

Justin Hill, MD, Burke Rehabilitation Hospital, has received a $48,000 grant from the Craig H. Neilsen Foundation to support a pilot program aimed at helping brain injury patients recover using Pt Pal, an app that allows therapists and physicians to prescribe and monitor home exercise programs. A key feature of the pilot program, Pt Pal provides education, coaching and support directly to patients via their smartphones, encouraging their home therapy program and shortening their rehabilitation period. The app also enables physicians, therapists and other clinicians to communicate with patients and manage care between visits.
"Rehabilitation is an extensive process, and keeping patients motivated and supported is important," says Matthew Bartels, MD, Professor and Chairman of Rehabilitation Medicine, Montefiore.
Montefiore is the first to implement Pt Pal on such a broad scale. It is being rolled out in some subspecialties, including Cardiac Rehabilitation and Women's Health Rehabilitation. The app includes appointment reminders and can track patient compliance with exercise programs, record biometrics like daily steps and blood pressure, and provide a video coach to demonstrate exercises.
On November 9, Montefiore Care Management (CMO) will transition from the CCMS system to Epic. With a network of more than 3,000 providers, the Epic Healthy Planet care management conversion is especially significant as it makes it easier for providers to refer patients for care management and for extended care teams like nurses, pharmacists and social workers and to view created Montefiore Care Management care plans in Epic.
