Collaboration to Focus on Improved Quality, Convenience and Efficiency of Care  

NEW YORK (March 9, 2017) – Montefiore Health System and CityMD are proud to announce a new partnership that will improve access to primary, specialty and urgent care services for patients of both CityMD and Montefiore. Under our agreement, Montefiore will become CityMD’s exclusive preferred provider partner in the Bronx and Westchester. Individuals who visit CityMD’s urgent care centers in these communities will now have the opportunity to be seamlessly connected to necessary follow-up care with Montefiore’s world class community of primary and specialty care providers.

This partnership ensures appropriate follow up care as needed, without disruptions. Sixty percent of CityMD urgent care patients do not have primary care physicians and many suffer from chronic conditions such as diabetes, asthma and heart disease. CityMD and Montefiore will offer patients a pathway to a positive experience, timely care and ultimately, optimized health and well-being. Working together, the two organizations’ transformative healthcare practices will achieve these joint goals.
