Please carefully review the following information BEFORE your first visit. There are two documents which you must download and print out BEFORE your first visit. If you have difficulty in downloading or printing these documents please contact our office and we can send them to you via US mail or e-mail.
Your First Visit to the Montefiore Headache Center
As part of your first visit to the Montefiore Headached Center, we ask that you download and complete the following questionnaire and headache diary prior to your appointment to help us understand your headache pattern, pain characteristics, and medical history. Please download and complete the Headache Diary over the next 30 days BEFORE your first appointment and bring it and the questionnaire with you to your appointment. Also bring a complete list of your current and past medications, related exams, and other medical information which may be helpful in understanding and treating your headaches. All of this information will be reviewed during your first visit.

During your initial visit to the Montefiore Headache Center, a physician will interview you about your headaches, obtain a complete medical and health history, conduct a physical and neurological examination, and discuss how headaches affect your life. Based on the results, your physician may recommend additional tests and/or an interview with our Clinical Health Psychologist. Your physician will then combine all information to develop an individualized, comprehensive, multidisciplinary headache management plan specifically for you.

HIPAA
To ensure your privacy and confidentiality, we ask that you review the Health Insurance Portability and Accountability Act of 1996 (HIPAA) Joint Privacy Notice Summary (click on the link below). You will be asked to sign a document stating that have read and understand these policies at the time of your first visit.
