Montefiore Accountable Care Organization - ACO

The Montefiore Accountable Care Organization (ACO) is an initiative that provides certain beneficiaries of the original Medicare program access to enhanced care coordination programs and a focus on illness prevention and wellness.

In addition to Montefiore's 11 hospitals in the Bronx, Westchester and Orange and Rockland counties and its extensive primary care and specialty sites and its employed and affiliated community-based physicians, the Montefiore ACO includes providers from other healthcare organizations in the Bronx, Brooklyn, Staten Island, Westchester and the Hudson Valley

Government Innovation Models Participation

HISTORY

Montefiore has been providing coordinated care services to patients across multiple care settings for more than two decades and has been an ACO since 2012. The Montefiore ACO participated in the Next Generation ACO initiative in 2017 following the end of the Pioneer ACO model. Participation in the Pioneer ACO program concluded as of December 31, 2016.
Participation in the Next Generation ACO program concluded on December 31, 2019.

NEW INITIATIVES

In 2020, Montefiore ACO was accepted to participate in the Implementation Period of the Direct Contracting Model  which runs from October 2020 – March 2021. Direct Contracting (DC) is a voluntary risk sharing payment model aimed at reducing total cost of care expenditures and preserving or enhancing quality of care for beneficiaries in Medicare FFS.
