Montefiore serves as a tertiary care referral center for patients from across the New York City metropolitan area and beyond, and offers a full range of healthcare services to two million residents throughout the neighboring counties of Westchester and the Bronx, one of the most diverse and poorest urban communities in the United States.
Comprised of four hospitals with 1,491 beds, Montefiore has 93,000 annual hospital admissions. Connected by a robust health information technology system (HIT), Montefiore provides more than 2.6 million ambulatory visits through a network of nearly 100 locations - from community-based ambulatory care centers to school-based health centers to mobile clinics. With nearly 300,000 visits per year, Montefiore's emergency department is one of the busiest in the nation, while the home health program provides over 500,000 visits annually. Forty percent of revenues from inpatient discharges stem from Medicare, 37 percent from Medicaid and 23 percent from commercial and other insurance. Montefiore's approach has yielded a financially stable and flourishing organization that produces high-quality outcomes.
Montefiore has also created a strong alignment with its practicing physicians-more than 1,600 of whom are employed by the medical center-to provide high-quality, cost-effective care; promote community health; and improve patient outcomes.
Montefiore has developed and implemented interdisciplinary programs focused on the management of chronic diseases to elevate patients' quality of life, reduce hospitalization and manage costs. The Montefiore Independent Practice Association (MIPA) accepts pre-payment from insurers, assuming responsibility for meeting the members' complete healthcare needs, and being accountable for quality and finances. Montefiore has created a Care Management Organization (CMO) to manage the program and care for the population of 150,000 enrollees. Enabled by advanced HIT, such as telehealth and patient portals, the CMO serves as an incubator for the creation of new and innovative approaches to improve transitions of care and care management and coordination, tailored to the particular needs of patients who have complex medical and social conditions.
Embracing its social responsibility to the community, Montefiore is nationally known as a pioneer in programs that are tailored to the specific needs of the community. Our community mission is our legacy. We have been in the vanguard of interventions to combat such conditions as HIV disease, tuberculosis and lead poisoning prevention. Recently we have sharpened our focus on such issues as childhood obesity, diabetes, improving community access to fresh, healthy foods at green markets and reducing healthcare disparities. We are aligning components of our delivery system to help improve public health outcomes and building our behavioral and population-based research to identify best practices. Montefiore has had considerable successes partnering with governmental and community-based organizations to reform policies to improve the health and well-being of our communities. Recent examples include taking the lead in advocating for the use of reduced fat milk in the schools and spurring legislation requiring all patients be educated about breast reconstruction after mastectomy to reduce the disparities found to exist between low- and high-income patients.
