
The Division of Nephrology of the Department of Medicine established the Montefiore Kidney Care Program to enhance the treatment of patients with chronic kidney disease (CKD). We are committed to providing our patients with the highest quality care possible to reduce complications of kidney disease through intensive management, patient education, and efficient communications with our patients' primary care doctors.
More than 500,000 Americans require dialysis or transplantation due to kidney failure and nearly 20 million adults across the nation demonstrate some evidence of chronic kidney disease. Most individuals amidst the early stages of kidney disease do not experience symptoms or feel sick at all. Unfortunately, the majority of patients with kidney disease are unaware they have a disorder until they've already lost a sizable portion of their kidney function.
So what causes kidney disease? The primary risk factors include: diabetes, high blood pressure, heart disease and a family history of kidney disease. Hence, by controlling heart and blood vessel disorders, and treating complications from kidney disease, patients can slow their CKD and reduce their risks of cardiovascular events. Some of these control measures include: maintaining normal blood pressures, reducing blood sugars, quitting smoking, managing cholesterol, regulating blood mineral levels, treating anemia and having regular checkups to monitor vitals and the progression of CKD.
Dedicated Chronic Kidney Disease Care Team
We realize a diagnosis of kidney disease can be very troubling; however, at the Montefiore Kidney Care Program, our staff is specially trained in diagnosing and managing kidney diseases. Working in a multidisciplinary fashion, we forge partnerships between patients and specialized medical, nutritional, educational and social service experts in order to maximize their care experience.
The Montefiore Kidney Care Team is comprised of: a nephrologist (a physician who specializes in kidney diseases), a registered dietician who discusses dietary changes with patients, a social worker to help with patient education and insurance issues, a licensed nurse who monitors blood pressure and administers vaccines, and a care coordinator who makes all the necessary appointments and referrals. Patients are not alone in their battle against kidney disease. We formulate a plan so each patient not only receives treatment, but also through education and counseling becomes an active participant in his or her own kidney health.
