Montefiore Home Care provides the following skilled, clinical services and programs:

• Telehealth for congestive heart failure and diabetes
• HIV/AIDS Specialty Care
• Social Work Services

Once a skilled service has been arranged, our dedicated team will assist patients with coordination of other services such as home health aides and in-home lab services. Many of our healthcare providers speak both English and Spanish, and our staff is sensitive to the unique needs of people’s different cultural backgrounds.
