Clinical Services
Our dedicated and highly trained registered nurses will work closely with you and your healthcare team by keeping you on track for recovery by developing your personal home healthcare plan. They will monitor your vital signs, educate you and your family/caregiver on administering medications and teach you and your caregiver how to control the impact of acute and chronic illnesses through continuous education and reinforcement of symptom management.
Our nurses will also communicate your progress and healthcare needs to the members of the Montefiore Home Care team as well as to your referring physician.

Wound Care
Our wound care specialists and registered nurses are committed to ensuring a positive healing outcome for pressure, traumatic, surgical, venous, arterial and venous wounds. Our Wound Care Team works closely with your physician and will also educate you about the nature of your wound and how it should heal to achieve a positive outcome.
