HIV/AIDS Home Care Program

The Montefiore HIV/AIDS Home Care Program provides quality care to people living with HIV/AIDS related illness, all in the comfort of the home. Our specially trained professionals are dedicated to providing skilled clinical services, and will make periodic home visits, create a plan of care, and work closely with the entire medical team to ensure you receive the best care possible.

Our services include:

• A personal treatment program to help you become knowledgeable about all aspects of your disease and treatment
• Education to teach personalized medication scheduling, pill pouring and side effects
• Highly trained registered nurses
• Social work services
• Rehabilitation services (PT, OT, Speech Therapy)
