The following documents are mandatory and should have been submitted to your department once accepted to the program. If you did not submit any of the following, please submit along with the required forms to the House Staff Office.
- Current Transcript
Must be an original mailed from your medical school to the House Staff Office. - Curriculum Vitae
- Must be current and updated.
- If you submitted one via ERAS please resubmit.
- Recommendation Letters
- Three are required.
- Must be within a year of contract start date.
