Documents

The following documents are mandatory to start your residency/fellowship and should be submitted immediately except for your final transcript and diploma, which are due AFTER you graduate.

1. Final Transcript
   1. Must be an original mailed from your medical school to the House Staff Office.
   2. If you are a foreign medical graduate a notarized copy is accepted.
   3. Must say DEGREE CONFERRED
2. School Diploma
   A copy is sufficient.
3. Curriculum Vitae
   1. Must be current and updated.
   2. If you submitted one via ERAS please resubmit.
4. Recommendation Letters
   1. Three are required.
   2. Must be within a year of contract start date.
