The following documents are mandatory to start your residency/fellowship and should be submitted immediately.
- Final Transcript
- Must be an original mailed from your medical school to the House Staff Office.
- If you are a foreign medical graduate a notarized copy is accepted.
- Must say DEGREE CONFERRED.
- Medical School Diploma
A copy is sufficient. - Verification of Post Graduate Training
- Copies of any certificate of Training.
- Letter from old/current program director(s).
Must include start and end dates.
- Curriculum Vitae
- Must be current and updated.
- If you submitted one via ERAS please resubmit.
- Please account for all time after you have graduated from medical school. For example, 2002-2004 residency in internal medicine, 2003-2004 backpacked through Europe, 2004-2005 residency in internal medicine.
- Recommendation Letters
- Three are required.
- Must be within a year of contract start date.
- ECFMG Certificate
- Foreign medical graduates ONLY!
This includes foreign medical schools with US campuses. - Must be NOTARIZED.
- Foreign medical graduates ONLY!
- Life Support Certificates (if applicable, please check with your department)
- BLS
- ACLS
- PALS (if working with children)
- NYS Licensure (Please check with your department for licensure requirements.)
- Medical
- For non-accredited programs

- Dental
- For all dental residents
- Must obtain either a License or Limited Permit
