Documents

The following documents are mandatory and should have been submitted to your department once accepted to the program. If you did not submit any of the following, please submit along with the required forms to the House Staff Office.
 

1. Final Transcript
   1. Must be an original mailed from your medical school to the House Staff Office.
   2. If you are a foreign medical graduate a notarized copy is accepted.
   3. Must say DEGREE CONFERRED.
2. Medical School Diploma
   A copy is sufficient.
3. Verification of Post Graduate Training
   1. Copies of any certificate of Training.
   2. Letter from old/current program director(s).
      Must include start and end dates.
4. Curriculum Vitae
   1. Must be current and updated.
   2. If you submitted one via ERAS please resubmit.
   3. Please account for all time after you have graduated from medical school.  For example, 2002-2004 residency in internal medicine, 2003-2004 backpacked through Europe, 2004-2005 residency in internal medicine.
5. Recommendation Letters
   1. Three are required.
   2. Must be within a year of contract start date.
6. ECFMG Certificate
   1. Foreign medical graduates ONLY!
      This includes foreign medical schools with US campuses.
   2. Must be NOTARIZED.
7. Life Support Certificates (if applicable, please check with your department)
   1. BLS
   2. ACLS
   3. PALS (if working with children)
8. NYS Licensure (Please check with your department for licensure requirements.)
   1. Medical
      1. For non-accredited programs
      2. To obtain an application log on to http://www.op.nysed.gov/ 
   2. Dental
      1. For all dental residents
      2. Must obtain either a License or Limited Permit
      3. To obtain an application log on to http://www.op.nysed.gov/
