Employment Eligibility

You must provide two personal identifications with your package. For example, a copy of your social security card and your driver's license.

1. Social Security Card 
   Mandatory for all
   1. You must provide a copy of your social security card
   1. If you have lost of misplaced your card, you will need to reapply for one and send us a copy of your application.
   2. If you are a foreign medical school graduate, not living in the United States, you will have to apply for one upon arrival in the country.
   2. You will not be put on payroll or direct deposit until we receive a copy of the card
   3. If you need to apply please obtain a letter from the House Staff Office

2. U.S.A. Passport or Driver’s License 
   Mandatory if you are a US Citizen
3. Permanent Resident Card or Alien Card (if applicable)
4. Current Visa -- (if applicable)
   1. Current I-94
   2. Copy of Visa
5. Employment Authorization Card (if applicable)
6. US Naturalization Certificate – Non-U.S. Born (if applicable)
