Post Graduate Year 1 (PGY 1)

Please print or type clearly on all applications and submit all required documents, employment eligibility documents and forms to the House Staff Office, not to your department.  All documents downloaded and completed from the website must be mailed directly to the House Staff Office.  If you would like to come in person to submit your documents and forms, call for an appointment.

• Please note that we must receive completed forms and required documents in order to put you on payroll.
• Your contract for the 2008-2009 year, along with our background information form, will be emailed to you. Please sign both and return it to the House Staff Office.
• If you are on a VISA please call the House Staff Office immediately to start the process.
• In the event that your current address changes before July, please submit a Change of Information Form immediately.
• In the event that your email address changes please notify the House Staff Office immediately.
