Coordinated Care Management (CCM)

At Montefiore, Coordinated Care Management (CCM) is a professional model of care delivery in which each patient's care plan is evaluated and implemented by an interdisciplinary team. Nurses, as the point-of-care providers, coordinate each patient's care plan with his or her physician. Professional nurses integrate, coordinate and facilitate the delivery of healthcare.

Quality Management

At Montefiore, Nursing Quality Management (QM) is consistent with the philosophy, mission and goals of Montefiore. Our program is customer-focused, interdisciplinary, data-driven, outcome-oriented and proactive. We adhere to continuous, ongoing performance evaluations, using the plan-do-check-act (PDCA) model, to objectively and systematically monitor and evaluate the quality and appropriateness of all nursing care.

Using Technology

Montefiore nurses benefit from the medical center's sophisticated Clinical Information System (CIS), which provides access to cutting-edge technology designed to enhance the quality of patient care. The CIS includes an electronic medical records system, which enables nurses to visualize a patient's plan of care based on physicians' orders and eliminates paper charting. Medications are documented on wireless portable input devices. Internet and intranet access to pertinent clinical resources, including journals and reference databases, is available on patient units.

Research

Since critical thinking is essential to nursing, Montefiore nurses receive extensive training in developing, implementing and presenting research projects on many subjects. Recent projects include the evaluation of e-testing, and the examination of a critical care predictive model, using qualitative and quantitative data. Developed 20 years ago by Montefiore, this model determines which patients would or and would not benefit from ICU admission. Our nursing staff are also well represented on Montefiore's institutional review board.
