Professional Practice

Professional nursing practice at Montefiore Medical Center is an exemplar of excellence in the delivery of Patient/Family Focused Care.  This is accomplished through professional collaboration and team cohesiveness while partnering with our customers and community to improve health and wellness.

Coordinated Care Management (CCM)

At Montefiore, CCM is the professional interdisciplinary model of care delivery. This model ensures that each patient's written care plan is implemented and evaluated by an interdisciplinary team.  Nurses are the point of care providers. They are best able to coordinate each patient's care plan with his or her physician. Professional nurses integrate, coordinate and facilitate the delivery of health care. CCM provides a clinical infrastructure that supports interdisciplinary clinical practices and processes in a managed care environment. 

Quality Management

At Montefiore, Nursing Quality Management (QM) is consistent with the philosophy, mission and goals of the medical center. Our QM program is customer-focused, interdisciplinary, data-driven, outcome-oriented and proactive. We adhere to continuous, on-going performance evaluations, using the plan-do-check-act (PDCA) model, to objectively and systematically monitor and evaluate the quality and appropriateness of all nursing care within the system.

Using Technology

Montefiore nurses benefit from the medical center's sophisticated Clinical Information Systems (CIS), which provides access to cutting-edge technology designed to enhance the quality of patient care. The CIS enables nurses to visualize a patient's plan of care based on orders, thus eliminating paper charting and providing an electronic medical record. Medications are documented on wireless portable input devices. Internet and intranet access to pertinent clinical resources, including journals and reference databases, is available on patient units.

Research

Critical thinking is essential to professional nursing practice. Montefiore nurses receive extensive training in formulating, implementing and presenting research projects on a wide variety of subjects. Recent projects include evaluation of e-testing on pilot units, and evaluating a critical care predictive model using qualitative and quantitative data. The model was originally developed 20 years ago at Montefiore to determine who does and does not benefit from an ICU admission. The question about what has changed and what has stayed the same over the past 20 years will be answered. And nurses are well represented on the medical center=s IRB (institutional review board).
