CMO services spans the entire healthcare continuum, including hospital care, rehabilitation, outpatient care, professional services, home care, mental health counseling, community-based services, remote patient monitoring and many other programs. We strive to help patients understand their medical condition, follow their physician's treatment plans and medication regimens and participate in self-care activities. Our services are designed to proactively anticipate the individual needs of a patient and to provide the scope of necessary health services to return them to optimum health.
Because of our close affiliation with Montefiore Medical Center, we are able to refer our clients to a care network that includes 2400 credentialed providers. We also offer the expertise of well trained nurse care managers, telephone access to a knowledgeable customer service staff, and online health information. When needed, we will provide our patients with sophisticated telemonitoring devices or assist them in the comfort of their home through HouseCalls, our home visit program.

Managing ongoing treatment for a chronic condition can be a difficult and overwhelming task that involves keeping track of multiple appointments, taking many medications and coordinating several physicians. We can ease this burden by taking over management of a patient's care plan and offer remote family members the peace of mind that comes with knowing our staff is involved in every aspect of a patient's care.
For patients leaving the hospital, either to return home or to move to another facility, CMO has developed a number of key initiatives to support the hospital transition process, particularly for patients with post acute care needs.
CMO works with many of the leading health plan providers to offer services to their members, and is working closely with the Center for Medicaid and Medicare Services to provide care to high-risk beneficiaries in the Bronx.
