Vascular and Interventional Radiology

The ACGME-accredited Vascular and Interventional Radiology Fellowship of the Unified Department of Radiology of the Albert Einstein College of Medicine is based at Montefiore Medical Center.  The fellows are based at the Moses Division.  There are two one-year fellowship positions which cover a broad range of vascular and nonvascular diagnostic and interventional procedures.  The Section of Vascular and Interventional Radiology is an integral part of a Department that has a well deserved local and national reputation for teaching and clinical experience.

The goal of the fellowship is to provide a tailored teaching experience, giving the fellow greater responsibility as he or she progresses through the year.  Four Section attendings (Drs. Jacob Cynamon, Seymour Sprayregen, Andrei Frost and Jon Trambert) are responsible for teaching and supervision.  In addition to the fellow, one or two residents are assigned to the Section at all times.

An unusually close working relationship with the clinical services at Montefiore, including Vascular Surgery, Urology, Gastroentology, and General Surgery, helps provide a rich experience.  In addition to the excellent clinical material, VIR Section, departmental and inter-departmental didactic and case conferences enhance the fellowship experience.  For example, the Section actively participates in weekly Vascular Surgery Grand Rounds.  The CT and the Ultrasound Sections adjoin Vascular and Interventional Radiology, allowing for active case consultation between Sections.

As the fellow gains more experience, he or she plays an increasingly active role in the teaching of radiology residents.  There is an adequate volume of clinical material without "turf" battles between residents and fellows.


Work Schedule

The day generally begins at 7:30 AM, with work rounds on the patient floors, and usually concludes about 6:00 PM, often with a film review session.  Weekly and monthly conferences include Vascular Surgery Grand Rounds, and department Angio Case Conference, Angio Journal Club, Basic Principles of Vascular and Interventional Radiology, and Quality Assurance.  The fellows take call in rotation with Section attendings, and they have attending staff back-up. Radiology residents assist in all emergency cases.


Case Load

Approximately 1,450 cases are performed per year at Moses; about 700 cases are performed at Weiler. There is an emphasis on cardiovascular diagnosis and intervention with approximately 200 arterial and venous angioplasties performed yearly.  Transcatheter thrombolysis, embolization, intravascular stents, IVC filter placements and TIPS procedures are also performed routinely.  Fellows are also responsible for nonvascular interventional procedures, eg. biliary drainage and stents, percutaneous nephrostomy, ureteral stents and abscess drainage.  The Section also has assumed responsibility for peripheral MR angiography.  Fellows will have ample opportunity to participate in the stent graft program at Montefiore.

Because of the volume and variety of clinical material, there is ample opportunity for research and publication if desired.


Equipment and Space
There are two Philips angio rooms in the Section, a Poly A/U and an Integris V-3000.  Both rooms have full digital capability and each is equipped with a BCM long-leg changer.  The rooms are adjacent and share a common control room corridor.  A new Philips Neurorangiography suite is available to the Section on a part-time basis.   All angiography suites have full physiologic monitoring equipment.  There is a "site-rite" portable ultrasound dedicated to the Section to help access different vessels.  A Philips 1.5 tesla MR scanner is used for peripheral MR angio studies.  In addition, the Section has ready access to three nearby CT scanners and four color-flow Doppler ultrasound units.

Location
Montefiore Medical Center is located in a pleasant residential section of the North Bronx, close to the Westchester border and several major highways.  Residents, fellows, and Faculty easily commute from Manhattan, Westchester, Connecticut, Rockland, and New Jersey.

Montefiore housing is available in Riverdale (10 minutes away) and also on the Medical Center grounds.
