“Our goal is to provide quality primary care for the elderly where they need it, and that means offering state-of-the-art diagnosis and treatment in their homes,” said David Bernard, MD, medical director of the program at Montefiore. “In addition to delivering high quality long-term care, the program also offers urgent house calls on a same-day basis. We think this addresses the growing healthcare needs of an extremely vulnerable population,” said Dr. Bernard.
The Montefiore medical team consists of two physicians, two nurse practitioners, a social worker and key support staff.“By increasing access to primary care for the frail elderly we can also help prevent conditions that would bring these patients to the hospital with more serious conditions,” said Dr. Bernard.
Initially, the house call program will be available to eligible seniors in the Tremont, Melrose and Morrisania sections of the Bronx, whether or not they live in senior-funded housing, housing authority buildings or private homes and apartments.Montefiore Medical Center has established community partnerships as the foundation of this program with the New York City Housing Authority, Fair Pharmacy, Montefiore Home Health Agency and MetroCare Ambulance Service.
