Patient Care Coordinator

Currently, we are seeking a Patient Care Coordinator to join our astonishing team. In this position, you will coordinate the cost-effective delivery of high-quality care to our acute care patients. Responsibilities include assessing the healthcare needs of patients and their families, facilitating the development and implementation of interdisciplinary care plans, monitoring the performance of nursing personnel, evaluating the progress of each patient, establishing discharge plans, and participating in performance improvement initiatives. 

To qualify, you must have a BSN degree, current NYS RN licensure, and 5 years of clinical experience (2 of which must be in acute care nursing). Certification in acute care nursing is ideal.
