Family and Social Medicine – Patient-Centered Medical Homes – New York City – Montefiore Medical Center

The Department of Family and Social Medicine prides itself on utilizing a Patient-Centered Medical Home model of care, with the goal of delivering comprehensive, integrated healthcare to patients.

This unique model of care aims to improve access, population disease control, patient-self management, and the role of primary care physicians as coordinators and managers of care. In a medical home setting, a dedicated primary care physician oversees all aspects of the patients' care, throughout all stages of life.

This lead physician works collaboratively with any other health professionals a patient sees—from specialists to nurses to social workers—to ensure that care is consistent, efficient and personalized.

The Department of Family and Social Medicine's primary outpatient clinics, and our affiliate sites, meet the standards established by the National Center for Quality Assurances to qualify as Patient-Centered Medical Homes.
