Agreement Calls for Montefiore's CMO To Continue Managing Care of 90,000 EmblemHealth Members

New York City, NY (January 11, 2011) - Montefiore, the University Hospital and Academic Medical Center for Albert Einstein College of Medicine, and EmblemHealth have renewed their agreement for an additional three-year term effective on January 1, 2011.

The agreement calls for Montefiore, through its subsidiary, The Care Management Company (CMO), to continue to manage the care of  90,000 EmblemHealth members.  The CMO coordinates care and provides vital disease management services for these members, thus reducing costs and improving outcomes. 

Building on a unique ten-year partnership, Montefiore and EmblemHealth have also agreed to explore similar arrangements for EmblemHealth's fee-for-service patient population.  These arrangements will embrace the concepts of ACOs (Accountable Care Organizations), which focus on enhancing the patient experience and overall health and well-being, improving quality outcomes and reducing costs.
