Assisted Reproductive Technologies

At Montefiore's Institute for Reproductive Medicine and Health patients have access to a complete range of advance treatment options for men and women, including:

Assisted Reproductive Technologies (ART) are a variety of procedures developed to overcome infertility and improve the chance for conception. ART procedures involve the following steps: ovulation stimulation, egg retrieval, fertilization and embryo transfer.

In Vitro Fertilization (IVF) is a procedure in which eggs are retrieved by a non-surgical technique and combined with the sperm in the laboratory. The resulting embryos are transferred back to the uterus.IVF is used mainly to treat infertility that results when the woman's fallopian tubes are not functioning properly.IVF can also be used to treat otherwise unexplained infertility. All procedures are done on site including: ultrasound, estradiol monitoring, egg retrieval, and embryo transfer.

Intracytoplasmic Sperm Injection (ICSI) is a specialized form of IVF in which the egg is injected with a single sperm by using a microscopic glass needle. This new technique enables us to help couples create embryos even if the father has very few living sperm. The female partner of a couple needing ICSI is often more fertile than the average woman seeking fertility care. Pregnancy rates among ICSI couples are, therefore, often greater than the average for women seeking fertility care.

Testicular Sperm Extraction (TESE) is a procedure used when there are few or no sperm in the ejaculate. Under local anesthesia, a needle is passed into the testicule and a small core of tissue is obtained. Sperm or immature sperm cells are extracted from the testes by the embryologist. These sperm are used for the ICSI procedure.

Blastocyst transfer allows for extended culture of embryos to the blastocyst stage. The purpose is to reduce the risk of multiple pregnancies. Blastocyst stage embryos are thought to have a greater chance of establishing pregnancy. Therefore, it will allow for fewer embryos transferred to the uterus per cycle.

Cryopreservation is the technique of freezing embryos to be used for transfer in later cycles.

Assisted hatching is used to help the embryo emerge from its protein shell. A small slit is carefully made in the shell that makes it easier for the embryo to emerge. This slit may also improve transfer of signals from the embryo to the mother, increasing the chances of implantation.

Donor eggs - In 1986, the physicians at Montefiore Medical Center were among the first in New York to establish a donor egg program. Donors may be anonymous or known to the recipient. Anonymous donors are compensated for their time and effort. Our donor egg recipients have a greater pregnancy rate than the average IVF patient. We have no absolute age cut-off for recipients. Potential recipients over the age of 45 must go through medical clearance before being accepted to the program. We also require a preconceptional perinatal consult so the potential risks of pregnancy at an advanced reproductive age can be discussed. Our oldest patient to complete the program was 52 years old when her twins were delivered.

Sperm - Montefiore's Institute for Reproductive Medicine and Health is a New York State licensed center for the distribution of frozen sperm. We maintain relationships with licensed sperm banks across the country. Patients are given a profile of sperm donors to choose from.

Laboratory services - Montefiore's Institute for Reproductive Medicine and Health houses certified New York State licensed reference laboratories for both andrology (for semen analysis and sperm preparation) and endocrinology (for analysis of hormones such as FSH and Estradiol). Our laboratory services are available seven days a week.
