Before Your Visit

• Gather information - On the day of your appointment, bring the following with you
  • Referral/Authorization (if necessary) from your primary care physician's office.
  • Medical Records: MRI, CT, X-RAYS, EEG, other study reports and copies of medical records.
• Please arrive 15 minutes prior to your appointment for registration and check-in.
• If you cannot keep your appointment, please contact our office within 24 hours prior to the scheduled date.
• If you have any questions, please feel free to call.  We look forward to seeing you in our office.
